Finding a sudden, completely bald, coin-shaped patch on your scalp can be terrifying. This condition is called Alopecia Areata. It is not caused by stress, shampoo, or hard water—it is an autoimmune disease where your own body mistakes your hair follicles for foreign invaders and attacks them. The good news is that the hair follicles are usually not destroyed, they are just "asleep." With aggressive, targeted medical therapy, we can suppress the immune attack and wake the follicles back up for full regrowth.

Understanding Alopecia Areata

Alopecia Areata behaves very differently from standard male or female pattern baldness. It is unpredictable and can happen overnight to anyone, at any age, including children.

How it Presents

  • The Classic Sign: One or more perfectly smooth, round, completely bald patches on the scalp. The skin looks normal, not red or flaky.
  • Exclamation Mark Hairs: Under a dermatoscope, we see tiny, broken hairs at the edge of the patch that look like exclamation marks (!). This tells us the immune attack is active and the patch will likely grow.
  • Nail Changes: Some patients also develop tiny dents (pitting) or ridges on their fingernails.

Progression Risks

While a single patch may regrow on its own over several months, the condition is highly unpredictable. Without treatment, it can rapidly progress to Alopecia Totalis (complete loss of hair on the scalp) or Alopecia Universalis (complete loss of all body hair, including eyebrows and eyelashes).

Advanced Clinical Treatments

Home remedies like rubbing onion juice or garlic on the scalp are highly irritant and will often cause severe contact dermatitis, making the hair loss worse. We use evidence-based immunology to treat this condition.

Treatment Method How It Works Best For
Intralesional Corticosteroids Tiny micro-injections of medicine directly into the bald patch to locally suppress the T-cell immune attack. First-line treatment for adults with a few isolated patches. Highly effective.
Topical Immunotherapy / Steroids Potent medical creams applied at home to alter the local immune response over time. Children who cannot tolerate injections, or patients with widespread patching.
Systemic Therapy (Oral Meds) Oral corticosteroids or newer JAK-inhibitors that systemically calm the overactive immune system. Severe, rapidly progressing cases (Alopecia Totalis).
Doctor Advice

Dr. Manali's Advice

"If you spot a bald patch, do not wait to see if it fills in. The earlier we inject the patch to shut down the inflammation, the faster the hair regrows and the lower the chance of the patch spreading across your entire scalp. It requires patience—usually 4 to 8 weeks to see initial fine white hairs—but the success rate of clinical treatment is excellent."

Frequently Asked Questions

Understanding autoimmune hair loss.

Is Alopecia Areata contagious?

No, it is 100% an internal autoimmune issue. You cannot catch it from someone else, nor can you give it to anyone by sharing combs or hats.

When the hair grows back, will it be white?

Often, the first hairs to grow back in the patch will be fine and white. This is completely normal. The pigment-producing cells take a bit longer to recover, but the normal hair color usually returns within a few months.

Can PRP therapy cure Alopecia Areata?

PRP is excellent for stimulating growth, but it does not stop the immune system from attacking. Therefore, we always use medical treatments (like intralesional steroids) first to stop the attack, and may use PRP later to help speed up regrowth once the condition is stable.
