Why Salons Fail & Why Surgery is Permanent
To understand why your ingrown toenail keeps coming back, you have to understand the anatomy of the nail root (the matrix). If you simply trim the edge, the root is still alive and will push new, sharp nail into the swollen skin.

The Procedure: Partial Nail Avulsion (PNA)
This is a standard dermatological surgery performed right in our clinic under local anesthesia. It takes about 20-30 minutes and you can walk home afterward.
-
Step 1: Painless Anesthesia
We use a specialized nerve block technique at the base of the toe. You will feel a small pinch, and then the entire toe becomes completely numb. You will feel zero pain during the procedure.
-
Step 2: Removal of the Ingrown Edge
We do not remove the whole nail. We only remove the tiny 2-3mm sliver on the side that is digging into the flesh. Your nail will look slightly narrower, but completely normal.
-
Step 3: Chemical Matricectomy (The Permanent Fix)
We apply a precise chemical (Phenol or TCA) deep into the pocket where the root lives. This permanently destroys the matrix cells on that specific side. Without a root, the nail cannot grow back into the skin.
-
Step 4: Dressing and Healing
A sterile bandage is applied. We provide specific instructions for dressing changes at home. Healing takes a couple of weeks, but the pain relief is immediate once the numbing wears off.
Dr. Manali's Advice
If your toe is currently red, swollen, and oozing pus, do not try to dig it out yourself. This is a staph infection. We will likely start you on oral antibiotics to calm the infection down for a few days before performing the permanent surgical correction. Proper footwear (wide toe-boxes) is essential after recovery.
