Identifying the Infestation
Proper diagnosis is critical. Treating scabies with standard allergy medication will only provide temporary relief while the mites continue to multiply under the skin.
1. Scabies (The Human Itch Mite)
Scabies is caused by a microscopic mite (Sarcoptes scabiei) that burrows into the top layer of the skin to live and lay eggs.
- Key Symptoms: Extremely severe itching that is noticeably worse at night. Rash typically appears in skin folds: between the fingers, wrists, armpits, waist, and groin.
- The Danger: It is highly contagious through prolonged skin-to-skin contact. If one person has it, the entire household must be treated simultaneously.
2. Bed Bug Bites
Unlike scabies, bed bugs do not live on humans; they live in mattresses and furniture and feed on blood at night.
- Key Symptoms: Red, itchy welts that often appear in a line or a zigzag pattern ("breakfast, lunch, and dinner" sign) on exposed skin areas like the face, neck, and arms.
3. Head Lice (Pediculosis Capitis)
A very common issue in school-aged children. Lice feed on blood from the scalp and attach their eggs (nits) firmly to the hair shafts.
- Key Symptoms: Intense scalp itching and a tickling feeling of something moving in the hair. Nits look like tiny white dots near the scalp that cannot be easily brushed away like dandruff.

Eradication & Treatment Protocols
Medical treatment requires treating the patient AND the environment simultaneously to prevent re-infestation.
| Condition | Medical Therapy | Environmental Action Required |
|---|---|---|
| Scabies | Prescription Permethrin 5% cream (applied neck down) or oral Ivermectin, combined with potent anti-itch medication. All close contacts must be treated. | Wash all clothes and bedding used in the last 3 days in hot water and dry on high heat. |
| Bed Bug Bites | Topical corticosteroids to rapidly reduce swelling and antihistamines to control severe itching. | Professional pest control is required to eradicate the bugs from the home/mattress. |
| Head Lice | Prescription topical pediculicides (lotions or shampoos) that kill live lice, followed by meticulous nit combing. | Wash pillowcases, hats, and hairbrushes in hot water. |
Dr. Manali's Advice
"The biggest mistake patients make with Scabies is not treating the whole family. Even if your spouse or child isn't itching yet, they likely have the mites incubating. If only one person uses the cream, the mites will just pass back and forth indefinitely. Strict compliance with the treatment protocol is the only way to cure it."
