The Biology of Chemical Reconstruction
TCA Cross works by turning a deep, unhealable defect into an active wound that the body is forced to repair.
Which scars need TCA Cross?
TCA Cross is highly specific. It is used exclusively for:
• Ice Pick Scars: Deep, very narrow (less than 2mm wide) pits that look like a tiny ice pick punctured the skin.
• Boxcar Scars: Wider scars with sharp, steep edges that look like a box was pressed into the skin.
The "Frosting" Reaction
When the high-concentration acid is placed in the scar, the proteins in the skin immediately coagulate. This turns the tissue inside the scar stark white, a clinical endpoint known as "frosting." This proves the acid has penetrated to the necessary depth. Over the next few days, a small scab forms inside the crater. As it heals, fierce collagen production is triggered, lifting the floor of the scar higher than it was before.

Why Precision is Critical
TCA Cross uses acid concentrations ranging from 70% to 100%. If this touches normal skin, it will cause severe chemical burns and permanent scarring. This is why it must ONLY be performed by a board-certified dermatologist with extreme precision.
| Application Area | Clinical Outcome |
|---|---|
| Directly inside the scar (Correct) | Creates controlled injury at the dermal base. The walls of the scar contract and fuse, and new collagen pushes the base upwards, making the scar shallower. |
| Spilled onto healthy skin (Danger) | Causes a deep chemical burn that destroys healthy melanocytes and collagen, resulting in a permanent white spot (hypopigmentation) or a widened scar crater. |
Dr. Manali's Advice
"Your acne scars did not form overnight, and they cannot be rebuilt overnight. After a TCA Cross session, the scar will actually look slightly wider and redder for a few weeks before the new collagen matures and tightens the gap. You must trust the biological process and, most importantly, absolutely NEVER pick the scabs that form inside the scars."
